The people we support
Three core populations
Adults with intellectual or developmental disabilities (IDD)
We provide person-centered support for adults living with intellectual and developmental disabilities, including autism spectrum disorder, Down syndrome, cerebral palsy, and related conditions. Our services are designed to build independence, strengthen daily living skills, and connect individuals to the community resources they need to thrive.

Individuals experiencing housing instability
Stable housing is the foundation of wellbeing. We work with adults who are experiencing homelessness, at risk of losing their housing, or transitioning out of shelters, institutions, or incarceration. Our team helps navigate housing options, connect with assistance programs, and build the stability needed to move forward.

People navigating mental health or substance use challenges
We serve adults who are managing mental health conditions or recovering from substance use challenges and are ready to build a more stable, fulfilling life. We do not provide clinical treatment, but we walk alongside individuals as they pursue their goals — connecting them to the right resources, building life skills, and offering consistent, non-judgmental support.

Eligibility requirements
To receive services from New Horizons Wellness, individuals must meet the following criteria:
Age
Must be 18 years of age or older
Location
Must reside in the Dallas / DFW area
Insurance
Must have Medicaid coverage

Are you a family member or referral partner?
Families & caregivers
If you are supporting a loved one who could benefit from our services, we are here to help you navigate the process. We welcome calls and questions from family members and caregivers, and we will work with you to understand what support looks like for your specific situation.
Case managers & referral partners
We partner with case managers, social workers, discharge planners, and other professionals who refer clients to community-based services. If you are working with someone who meets our eligibility criteria, we welcome your referral and will respond promptly to coordinate next steps.
